We are a Salinas based hospice agency looking for a compassionate and dependable Registered Nurse to join our team. We are looking for individuals that understand the value of alleviating pain and suffering at end-of-life while meeting a shared goal of helping individuals live their final days with dignity and comfort. We currently have a need for a RN Case Manager for the Monterey area.Although a multi-location company, we pride ourselves on maintaining a local, family owned feel while providing exceptional care to those we have the honor of working with. We believe a work / life balance is not only important for the lives of our team members, but allows us to show up with the best care possible to those we serve.Whether you a seasoned hospice nurse or new to the idea of providing peace and comfort to those at end-of-life, we would love to hear from you.Essential Job Functions / ResponsibilitiesProvide in-home and facility visits to assess patient eligibility and needs to initiate appropriate interventionsDevelops care plan and teaches patient and caregivers care techniques as appropriateManage and administer medications as appropriateEducate and prepare patients, families and caregivers to end-of-life processesProvide mental and emotional support to patients and families as appropriateLead and collaborate with other members of inter-disciplinary team (Social Worker, Chaplain, CNA) providing patient care in conjunction with Hospice Medical DirectorCollaborate and communicate with referral partners as appropriateComplete accurate and relevant clinical notes in EMROur Ideal Candidates will be:Flexible and Adaptable to changeEffective communicator with strong interpersonal skillsEffective problem solverStrong assessment and analytical skillsOrganized and able to work autonomouslyReceptive and open to feedbackPassionate about what they do and why they do itRequired License and Qualifications:Registered nurse with current CA license Maintains current CPR Certification that meets American heart Association BLS GuidelinesLicensed driver with automobile that is insured in accordance with state requirementsAble to lift, position, and/or transfer patientsWe are invested in our employees and their well-being.
