Job Description:
Title Travel Physical Therapist – Skilled Nursing Facility
Schedule Sunday - Thursday or Tuesday - Saturday or Monday - Friday
Assignment Details Contract Length: 13 weeks Guaranteed Hours: 38
per week Requirements - Active CA PT License - SNF Experience
Preferred (Mentorship Available for 0-1 Years Experience) - Strong
Documentation and Time-Management Skills Roles & Responsibilities -
Evaluate and treat patients in a skilled nursing facility. -
Develop and implement individualized treatment plans to improve
mobility, strength, and overall functional independence. -
Collaborate with the interdisciplinary care team to enhance patient
outcomes. - Complete documentation in Net Health per facility
standards.
